Wegovy Titration Schedule

Wegovy (semaglutide for weight management) follows a 16-week titration before reaching the maintenance dose. Week 1-4: 0.25mg once weekly. Week 5-8: 0.5mg once weekly. Week 9-12: 1.0mg once weekly. Week 13-16: 1.7mg once weekly. Week 17 onward: 2.4mg once weekly (maintenance dose).

Each dose increase should be evaluated by your prescriber. If you experience intolerable side effects at any dose, your provider may extend that dose level for an additional 4 weeks before increasing. The goal is to reach 2.4mg while minimizing side effects, not to rush through titration.

Ozempic Dosing for Diabetes

Ozempic follows a simpler titration for type 2 diabetes. Start at 0.25mg once weekly for the first 4 weeks (this is a dose-initiation level, not therapeutic). Increase to 0.5mg once weekly. After at least 4 weeks at 0.5mg, may increase to 1mg if additional glycemic control is needed. A 2mg dose is available for patients requiring further A1C reduction.

While Ozempic is FDA-approved only for diabetes, the off-label weight loss at these doses is typically 6-10% of body weight, less than the higher Wegovy doses but still clinically meaningful.

Rybelsus (Oral) Dosing

Rybelsus is the oral tablet formulation of semaglutide. Start at 3mg once daily for 30 days (dose-initiation, not therapeutic). Increase to 7mg once daily. After at least 30 days at 7mg, may increase to 14mg once daily for additional benefit.

Critical administration requirements: Take Rybelsus first thing in the morning on an empty stomach with no more than 4 ounces (120mL) of plain water. Wait at least 30 minutes before eating, drinking, or taking other oral medications. These requirements are essential for absorption, as oral semaglutide has very low bioavailability (~1%) and food or other beverages significantly reduce absorption.

Missed Dose Guidance

For weekly injections (Wegovy, Ozempic): If you miss a dose and the next scheduled dose is more than 2 days away, take the missed dose as soon as possible. If the next dose is within 2 days, skip the missed dose and resume your regular schedule. Do not take two doses to make up for a missed one.

For daily oral tablets (Rybelsus): If you miss a dose, skip it and take the next dose the following day at the regular time. Do not double up.

If you miss multiple weeks, consult your provider. You may need to restart at a lower dose and re-titrate to avoid severe GI side effects from restarting at a higher dose.

Injection Timing and Best Practices

Semaglutide injections can be given any day of the week, but pick a consistent day and stick with it. The injection can be administered at any time of day, with or without meals. Common injection sites include the abdomen (at least 2 inches from the navel), front of thigh, or upper arm.

Rotate injection sites to prevent lipodystrophy. Allow the pen to reach room temperature before injection. The needle is very fine (31 gauge) and most patients report minimal discomfort. If you need to change your injection day, you can do so as long as there are at least 2 days between doses.
