Standard Titration

Ozempic follows a straightforward titration for type 2 diabetes. Week 1-4: 0.25mg once weekly (initiation dose, not therapeutic). Week 5+: 0.5mg once weekly (first therapeutic dose). If additional A1C reduction is needed after at least 4 weeks at 0.5mg, increase to 1mg weekly. If further control is needed after at least 4 weeks at 1mg, increase to 2mg weekly (maximum dose).

Many patients achieve good glycemic control at 0.5mg or 1mg and don't need to reach the 2mg maximum. Your provider should assess A1C response and tolerability at each level.

Ozempic Pen Instructions

Ozempic comes in two pen types: a pen with 0.25mg and 0.5mg doses, and a pen with 1mg and 2mg doses. Attach a new NovoFine needle before each injection. Inject subcutaneously in the abdomen, thigh, or upper arm. Rotate sites weekly.

The pen can deliver multiple doses from the same device. Keep track of remaining doses and have your refill ready before your current pen runs out. Store unused pens in the refrigerator; an in-use pen can be kept at room temperature for up to 56 days.

Off-Label Weight Loss Dosing

Some providers prescribe Ozempic off-label for weight loss at the 1mg or 2mg dose. While these doses produce meaningful weight loss (~6-10%), the FDA-approved weight management product Wegovy can titrate up to 2.4mg for maximum efficacy (~15% weight loss). If weight loss is your primary goal, discuss Wegovy or compounded semaglutide with your provider.
